This website and its contact forms are strictly for patient inquiries and healthcare provider consultations. This is a non-monitored channel for legal documentation.

We do not accept medical records requests, statutory notices, or legal subpoenas through this website. Any such submissions sent through this portal are considered invalid, are not reviewed, and will not be processed. Submission through this website does not constitute notice to Florida ID Care LLC or to any of its physicians or staff.

To submit a valid request for medical records, you must route it through our authorized channels:

  • Urgent or Expedited Requests: Must be sent via registered, verifiable post to the mailing address listed above.

All requests must include a valid, legally compliant patient authorization form.
